The Health Center, sometimes known as “The Nurse’s Office”, is coordinated by an Illinois Certified School Nurse (PEL-CSN) with the assistance of a Registered Nurse (RN) and a Licensed Practical Nurse(LPN).
Our goal is to bridge the gap between healthcare and education. Following a specific standard of practice, we offer a variety of student-centered services aiming to keep all EPCHS students healthy, safe, and ready to learn.

Health Records Requests:
Current EPCHS students can stop by the Health Center and request a copy of their immunization records. Former EPCHS students are required to use Parchment, a digital platform, to request their immunization records. Former students are charged a processing fee through Parchment in order to complete their request. Parchment allows its users to track requests from start to finish through their account.
